Community Care Programs
The community care program offers services in a community setting. A collaborative approach to health care is used to help manage your health.
Comprehensive Health Assessment Program (CHAP)
If you’re a carer or an adult with an intellectual disability, the CHAP is a questionnaire that documents your health history and provides your GP with prompts and guidelines relating to health issues. Service providers are able to register and download the document on your behalf.
