Linkages Program - Frankston and Mornington Peninsula regions

If you are under 65 or 50 for Aboriginal or Torres Strait Islander living with complex mental and physical health needs, the Linkages program may be able to assist you.

Local Area Coordinators (LACs)

Local Area Coordinators (LACs) can link you to the NDIS and to mainstream and community supports in your area. You can ask your LAC about the supports available in your community, even if you're not eligible for an NDIS support plan.

Peer to Peer Networks

Peer Networks are facilitated by families who meet regularly in the local community, bringing people together to learn and share knowledge, experience and connection in a friendly environment.

The Disability Trust

If you have an NDIS plan, the Disability Trust offers a range of programs and supports. They can help you connect with peers who are the same age and have similar interests.

Muscular Dystrophy NSW

Muscular Dystrophy NSW supports people living with neuromuscular conditions and their families.

They host events such as camps and retreats.
