Linkages Program - Frankston and Mornington Peninsula regions
If you are under 65 or 50 for Aboriginal or Torres Strait Islander living with complex mental and physical health needs, the Linkages program may be able to assist you.

Kundimia Community Home Support Program (CHSP) services are available to Aboriginal and Torres Strait Islander people living with disability in the Carnarvon region to assist them live independently at home.
Wathaurong - Home and Community Care
If you are under 50 years old with disability which restricts your ability to complete daily tasks associated with independent living, Wathaurong have a home and community care team that may be able to assist you.

Casa Services can help you with support packages to enable you to live in the community as independently as possible. They also assist with supported accommodation and respite.
