AgeWise Program

AgeWise delivers psychological therapies for older people with mild to moderate mental illness living in residential aged care facilities (RACF) in Tasmania.

Residential Recovery Program

Residential Recovery Program is open to adults who have a diagnosed mental illness. The program offers intensive recovery-focused support to participants to work on identified recovery goals in furnished and fully self-contained properties, providing a safe environment for participants to build capacity in living skills as they work to transition to independent living.

Mental Health Nurse Program

Richmond Futures provides Mental Health Nurse support for patients with a mental illness currently being managed by their GP or private Psychiatrist. The nurse will provide assessments, therapeutic interventions, medication monitoring, mental state assessment, family support, care coordination and referral to appropriate services.

Outreach Recovery Program

Richmond Futures' outreach recovery service is a one-on-one support program that aims to support participants to live in their own home or place of choice while connecting them with relevant services in their community.

Chorus - Psychosocial Support

Chorus offers community-based mental health support through one-to-one coaching and group activities for people living with severe mental health challenges.

360 Health and Community

If you have a serious, persistent mental health condition which directly effects your everyday functional capacity, 360 Health and Community may be able to help you build your support network and access psychosocial support services within your community.

Uniting’s Commonwealth Psychosocial Support program

This program helps people living with severe mental illness in the Western Sydney region to access short-term care to improve their wellbeing.

Miwatj Health

If you are 16 years or older and have a mental illness that impacts your daily life, Miwatj Health may be able to assist you.

Richmond Wellbeing

Richmond Wellbeing can assist people with severe mental illness build daily living skills and help you connect with services to meet your needs.

New Path - Getting back on track

Woden Community Service provides a mental health recovery program supporting people whose ability to manage daily activities and live independently in the community has been seriously effected by mental health issues.
