The Hospital to Home Program is a peer‑led service that supports people who have been in hospital due to experiencing mental distress to return home following a hospital stay.

Experts in Our Health

The Our Health project aims to build the knowledge, skills and confidence of Victorian women with disabilities to exercise their right to accessible health information and services, and to inform the delivery of health services in their communities.

The Friendship Project is designed to help young people with intellectual disability build skills and the confidence to make positive friendships.

Hunter Inclusion Project

Community Disability Alliance Hunter will develop peer groups across the Hunter region that will be peer led and support peers to be active citizens in their local community.

This program works with women living with disabilities through building confidence, connections and professional development.

Two Ways Mentoring Program

The Two Ways Mentoring Program is peer lead and aims to increase the economic and community participation of individuals living with mental health concerns.

Limbs 4 Life - Peer Support

Peer Support links individuals (and their families) pre or post amputation with trained amputees who have experience living with, and adapting to limb loss.

Circles Widen

This program supports the culturally and linguistically diverse (CALD) community of Western and South Western Sydney with social connection, and informal mentoring support to achieve goals.

My lived iD

My Lived iD is a hub for people with intellectual disabilities, their families, carers and health professionals. The hub provides resources and information to meet the needs of people with intellectual disability.

Active Abilities Series

Sport NSW has created a series of online workout sessions for people with disability. These have been created by athletes from across the physical, sensory and intellectual disability spectrum.
